Every member has their own unique profile page that contains a wealth of data on their health journey. Read below to learn more about what information you have access to.
Patient Health Journey
On entry, every member shows a high level overview of their health journey. This includes:
Name and ID
Age
Date of Birth
Past Medical History (shows to most recent three unique diagnosis)
Contact information (Email and Phone), if data available
Last Outpatient Visit, if data available
Last inpatient admission, if data available
Tags the member is in
Health journey timeline
Defaults to show the current year. Change the year by using the back and forward arrows
Click on a month to show details of prescription fills or medical usage for that month.
Star icon: This means this medication was filled for the first time for this member
90 icon: This means the member received a 90DS of this medication when filled

Patient details
Notes can be left in each members profile with the option of an attachment. 
Notes, once saved, are permanent and cannot be edited, however, amendments can be made on top of a permanently saved note
Notes are visible across all users in an organization, regardless of department.

Prescriptions are organized in three sections
1) Prescriptions: These refer specifically to prescriptions at CaryRx pharmacy. This is where you would manually select prescriptions to send to a member from inside OneDash
2) Pending Transfers: This will show a list of prescriptions for which transfer to CaryRx has been requested and is in the process of being moved over to the Pharmacy. Once the transfer is complete, the prescriptions will show in the prescription section. 
3) Other Prescriptions: This will list the full prescription history of the member, regardless of the location of the prescriptions
The view defaults to show paid prescription claims. You can use the toggle to show all claims, including reversed and rejected 
To add additional columns to your view, click on this icon and toggle on desired columns
To download this record offline, click this download button and receive a CSV of the data in this table. 
Search Bar: This search allows you to search by drug name or GPI. Begin typing and the table will narrow to match the drug name you have type or the GPI you have entered. 
You can request a prescription transfer manually directly from this table by selecting the prescriptions the member wants transferred and clicking on "Transfer Rx's to CaryRx"
If the patient has prescriptions being ordered from CaryRx, this tab will provide details on the status of those prescriptions and whether or not they have been delivered to the member.

All interventions identified for this member will be shown here. 
All interventions are visible across all users in the department, regardless of who the assigned owner(s) are.
Interventions are editable and actionable from inside this tab by clicking on the edit button under the actions column.
If an intervention is complete, you can view it and any notes that are inside it from the time of completion.  
Pause all interventions: If for any reason you would like this member to be excluded from all rule searches, you can put them on an indefinitely pause using this button.
Add Intervention: If you want to create a new intervention on the patient, you can do so manually using this button.
If you create a manual intervention using a rule that runs automatically, it will not pull in any custom variables it normally does so when it runs.
Every action completed on the patient, whether manual or automated and regardless of who performed it, will show here.
If an action was a sent fax, you will be able to download a PDF of the fax that was sent under the action column
If an action was a text message, you can hover over the information and see what message was sent to the provider
The CMR module provides a way to record and track information when speaking with the member about their medications. 
Click "create new" to start a new CMR
Allergies can be entered and saved
Current medical history defaults to pull ICD codes from the member's profile on the very first CMR. Clean up the list or add new diagnosis to accurately reflect members current medical history
Prescriptions will pull in on the first CMR from the prescription history. All fields are editable on double click
Patient verification is to record who you are speaking to
Clinical Expert Signoff is to record who completed the CMR
Saved CMR data is pulled into AI MTM evaluations so that evaluations are based on the most recent accurate prescription list.

These tabs provide a detailed view of the patient's medical claims, lab results, and inpatient usage.
All tables can be downloaded using the download button on the top right corner
You can filter the table by a specific ICD code and/or by a date range.
On default - all historical records are shown. To limit the table, be sure to add a start date filter
