Use this page to track all your patients/populations of interest. Learn more about each section of this page below.
Follow a Patient
Track an individual patient of interest and add them to your “my population” list for easy profile access. Clicking on the member name from this list will take you to that patient's individual profile.
Search for a Patient
Find a patient quickly by typing in their first name, last name, or their member ID, and the table will automatically narrow the list down to your search text
Access Patient Profile
Click on a patient name to view their profile and access prescription information, lab results, medical claims data, and AI medication recommendation results, if any.

Tags are a way to label a select population of interest. Tags can be made on any group of people. Think of them as a way to label a group of patients.
Create a Tag
Create a new tag for any group of patients. Add patients individually or all at once by pasting a list of ID numbers into the free text field. You can choose to add patients to the existing list or delete out the list and add a brand new set of patients.
Follow a Tag
Existing tags can be added to your follow list for easy access to that patient population
Tag --> Plan
Turn a tag into an intervention plan. Provide a patient start date for the plan's intervention, and be able to see pre- and post- intervention financial and clinical outcomes
When loading multiple patients into a tag at one time, ID's pasted in should format with a space in between each ID number.
REMEMBER: Tags are fluid and there is no historical record of the change of patients in a tag. If a patient is removed from a tag, there won't be a record they were in that tag.
