report columns column description lob grouped line of business (pdp, mapd/egwp, dsnp) measure the medicarestars measure (e g , dm, ras, statin) action the outreach channel (e g , fax, text, ehr) successful gaps with action anytime number of successful gaps where this action type was used at any point total gap count with action anytime total successful gaps (for this lob + measure combination); used as denominator for the below percent calculation percent with action anytime % of gaps that had this action at any time successful gaps with action as latest number of gaps where this was the most recent (latest) action total gaps with action as latest total successful gaps (for this lob + measure combination); used as denominator for the below percent calculation percent with action as latest % of gaps where this action was the latest recorded overview this report summarizes the most recent successful outreach actions taken on pharmacy related care gaps across medicare stars adherence measures it evaluates both the presence of the action per care gap and whether that action type is the latest action overall the goal is to understand how different communication types (e g , fax, text, ehr) are used in closing care gaps, and how that varies by line of business (lob) and stars measure use case this report is intended to quantify what proportion of closed care gaps had a specific outreach method performed identify the most common action types associated with successful gaps compare action performance across lobs and measures track how often a given action was the most recent outreach compared to other actions definitions term description successful gap a care gap with an outcome of success auto or successful action type channel or mode of outreach (e g , text, fax, ehr, manual call) lob grouped normalized line of business ( pdp , mapd/egwp , or dsnp ) latest action the most recent outreach action attempted for the intervention filtering criteria date range the care gap creation date is between january 1 of the current year and current date departments/teams includes only gaps assigned to pharmacy teams or associated pharmacy caseworkers lobs restricted to dsnp , mapd , egwp , and nd51 (converted to pdp ) measures includes the following medicarestars measures medicarestarsdm medicarestarsras medicarestarsstatin medicarestarssupd medicarestarsspc medicarestarscob medicarestarspolyach logic summary identify pharmacy interventions filters gaps by team or assignee and ensures the outcome is successful select most relevant actions includes only "sent" actions and filters out unsuccessful manual calls (left voicemail, failed calls) group and count tallies the number of care gaps that had each action type, both ever and as the latest aggregate by lob and measure groups results to enable direct comparisons across business lines and measures notes manual call types such as "left voicemail" and "failed call" are excluded from the latest action results the denominator for percentage calculations varies by whether you're counting the action occurring at any point in the care gap life cycle or action occurring last counts are based only on interventions with a successful outcome and valid action history
